Once complete please save your filled out sheet as a CSV (Comma delimited) file.

Step 2 Fill out the sheet on your device (you may need to 'enable editing') with as many orders as you require.
Step 3 Save the filled out sheet as a CSV (Comma delimited) file, you may need to click yes to any questions or warnings when saving.
Step 4 Enter your name and email address then upload the .CSV file below.
